What it involves

Supported Independent Living gives you help with everyday life in your home, at the times you need it. Our support workers help with routines, meals and getting out into the community, while our registered nurses look after your mental and physical health, including medications. The aim is to build your independence, not replace it.

What is included

The things this support covers day to day.

  • Help with daily living, personal care, meals and keeping the home running
  • Medication support with registered nurse oversight
  • Mental health check-ins and help to recognise early warning signs
  • Building routines and skills for more independent living
  • Support to stay connected with family, friends and community
  • Overnight support where your plan funds it

Who it is for

People with psychosocial disability who have Supported Independent Living funding in their NDIS plan and want to live in Western Sydney.

What makes it different

Nurse-led supported living. The same small team stays with you, our nurses oversee your health, and you have a say in your routines, your support and who you live with.

How this support starts

01

Get in touch

You, your family or your support coordinator contact us. We’ll talk about what you’re looking for and check your NDIS plan includes SIL funding.

02

Meet, visit and plan

We get to know you, your health and your goals. We help you find a suitable home and meet the team and any housemates, and together we plan your move and support.

03

Move in, with the same team

Your team supports you through the move and stays with you. Our nurses oversee your health, and we review regularly as you settle in.

Common questions

The things people ask us most often about this support.

Yes. If you’re not sure whether you’re eligible, we can talk it through with you and your support coordinator.

No. SIL can be delivered in your existing home, in a shared arrangement, or in a new place if that is what you want. The support follows you.

Where you share a home, we involve you in decisions about housemates, and we’ll work to find a good fit.

We review with you regularly and adjust the roster. If your needs increase beyond what your current plan funds, we will help you gather the evidence for a plan review.

Our registered nurses oversee your health care and medications, and guide your support workers so your whole team works the same way.

Let's talk about the support you need.

Whether it’s for you or someone you love, send a referral or give us a call. We’ll come back to you within one business day with next steps, who your team could be and a realistic start date.
