For support coordinators, hospital discharge planners, GPs, families and self-referrals. Tell us what the person needs and we will come back to you about whether we can help and when we could start.
Who can refer
Anyone can start a referral, including the person themselves. You do not need to have everything ready before you get in touch.

- Hospital discharge planners, treating teams and nurse practitioners
- GPs and allied health
- Families, carers and participants themselves
What happens after you send it
A referral is the start of a conversation, not a commitment.
1. We read it and call you. We confirm we have received the referral and ask anything that is missing.
2. We check we are the right fit. If we cannot provide what is needed, we say so early and point you elsewhere.
3. We meet the person. No cost, no obligation, in their own home if that is easier.
4. We agree a start. Supports, times and who will be delivering them, written down before anything begins.
