Mental Health Community Nursing

Registered nurses who know you, caring for your mental and physical health at home and in your community.

What it involves

Your nurse gets to know you, your health and what a good day looks like for you. They help manage your medications, spot early signs that things are changing, and look after your physical health, keeping your GP and treating team up to date along the way. The aim is simple: to help you stay well, keep doing what matters to you, and avoid time in hospital.

What is included

The things this support covers day to day.

  • Medication support, including administration, prompting, Webster packs and checking for side effects
  • Regular check-ins on your mental health and wellbeing
  • Help to recognise early warning signs and follow your wellness or relapse prevention plan
  • Physical health checks, such as blood pressure, weight and blood glucose
  • Diabetes, wound, continence and catheter care where you need it
  • Post-hospital discharge follow-up
  • Clinical notes shared with your GP and specialists
  • Guidance for your support workers, so your team works the same way

Who it is for

People living with a mental health condition across Western Sydney who need nursing support to stay well at home, including NDIS participants with psychosocial disability funding. Many people we support also have physical health needs, and we care for both.

What makes it different

Nurse-led care from one small team. Your nurse and your support workers are part of the same team, so your health and your everyday life are looked after together, and you never have to retell your story.

Common questions

The things people ask us most often about this support.

Not for NDIS-funded nursing. If your plan includes nursing supports we can start after an assessment. Veterans: DVA nursing works differently. See DVA Community Nursing.

Usually Improved Daily Living under Capacity Building, or high-intensity daily activity supports under Core when a nurse provides hands-on clinical care. Your assessment decides which applies, not the provider you choose.

Yes. Care is delivered by registered and enrolled nurses with current AHPRA registration, and high intensity supports are delivered by staff with the relevant additional training.

Once we have your plan details and an assessment is complete, we’ll give you a realistic start date rather than an optimistic one.

Let's talk about the support you need.

Whether it’s for you or someone you love, send a referral or give us a call. We’ll come back to you within one business day with next steps, who your team could be and a realistic start date.
