Support Coordination
One on one help to understand your NDIS plan and use it well.

What it involves
We help you understand and use your NDIS plan, connect with the right providers, and build the confidence and skills to manage your own supports over time. That includes the unglamorous parts: chasing quotes, comparing providers and keeping everyone to the same set of facts.
What is included
The things this support covers day to day. If something you need is not on the list, ask us anyway.
- Understanding what is in your plan and what each budget can be used for
- Finding and comparing providers, and getting quotes
- Setting up service agreements and bookings
- Resolving problems when a service is not working
- Preparing evidence and reports for your plan review
- Building your confidence to manage more of it yourself
Who it is for
Participants who want guidance navigating the NDIS system and coordinating multiple providers and services.
What makes it different
A dedicated coordinator who acts as an advocate, keeping communication clear between everyone involved in a participant’s care.
How this support starts
You get in touch
Send a referral or call. Tell us about the person, their plan, and what they need help navigating.
We assess and match
We understand your plan, we meet you, review what your plan funds, and match you with the coordinator best suited to your goals and circumstances.
Support begins
Your coordinator steps in, and stays. We review with you regularly and adjust as things change.
Am I eligible
Eligibility depends on the scheme, and the referral process is a little different for each. Choose the one that applies to you.
Support Coordination is its own funded support, it needs to be written into your plan, usually under Capacity Building. Not every plan includes it, so it is worth checking with your planner or Local Area Coordinator if you are not sure whether yours does.
There are three levels: Support Connection, Support Coordination, and Specialist Support Coordination. Your plan will say which one you are funded for, and we will tell you plainly what that allows before anything begins.
We currently support self managed and plan managed participants. The management type changes how invoices are handled, not the support you receive.
You do not need to be the participant to talk to us. Family members, carers, GPs and planners can all make referrals, and we will speak with you about eligibility before anyone commits to anything.
If you are helping someone navigate their first NDIS plan, or a plan that has become more complex, a support coordinator can take a lot of that load off you. We will explain what is funded, deal directly with providers on the difficult conversations, and keep you in the loop rather than working around you.
Common questions
The things people ask us most often about this support.
There are three: Support Connection, Support Coordination, and Specialist Support Coordination. Your plan will name which one is funded. We can tell you what your plan allows before you commit to anything.
No. A plan manager pays your invoices and handles the money. A support coordinator helps you decide how to use the plan and connects you with providers. Many people have both.
Yes, and we do it often. Our job is to get you the right provider, not to fill our own roster. If another organisation is a better fit we will say so.
Tell us. We will raise it, document it, and if it cannot be resolved we will help you move to someone else without a gap in your support.
