Support Coordination
One-on-one help to understand your NDIS plan and make it work for you.

What it involves
We help you understand your NDIS plan, find the right providers and keep everyone working together. That includes the unglamorous parts: chasing quotes, comparing providers and keeping everyone on the same page. If another provider suits you better, we’ll tell you.
What is included
The things this support covers day to day.
- Understanding what is in your plan and what each budget can be used for
- Finding and comparing providers, and getting quotes
- Setting up service agreements and bookings
- Resolving problems when a service is not working
- Preparing evidence and reports for your plan review
- Building your confidence to manage more of it yourself
Who it is for
People with psychosocial disability funding across Western Sydney who want help to navigate the NDIS and coordinate their supports, whether that’s one provider or several.
What makes it different
Your coordinator understands mental health and works closely with your nurse and support workers, while staying focused on what’s right for you, not on filling our roster.
Common questions
The things people ask us most often about this support.
There are three: Support Connection, Support Coordination, and Specialist Support Coordination. Your plan will name which one is funded. We can tell you what your plan allows before you commit to anything.
No. A plan manager pays your invoices and handles the money. A support coordinator helps you decide how to use the plan and connects you with providers. Many people have both.
Yes, and we do it often. Our job is to get you the right provider, not to fill our own roster. If another organisation is a better fit we will say so.
Tell us. We will raise it, document it, and if it cannot be resolved we will help you move to someone else without a gap in your support.
Let's talk about the support you need.
Whether it’s for you or someone you love, send a referral or give us a call. We’ll come back to you within one business day with next steps, who your team could be and a realistic start date.
