What it involves
We help participants build genuine connections within their local community, linking them to social groups, activities and networks that support belonging and inclusion. The aim is a place you keep going back to, not a one-off outing.
What is included

Am I eligible
Eligibility depends on the scheme, and there referral process is a little different for each person. Choose the one that applies to you.
Community Partnership is usually funded under Social and Community Participation, part of your Core Supports. Most plans include some funding here, but how much depends on your individual goals and circumstances.
You do not need a doctor’s referral to start. We confirm what your plan covers before anything begins, and tell you plainly if it does not cover what you are asking for.
We currently support self managed and plan managed participants. The management type changes how invoices are handled, not the support you receive.
You do not need to be the participant to talk to us. Family members, carers, support coordinators, and planners can all make referrals, and we will speak with you about what is possible before anyone commits to anything.
If someone you support has become isolated, or has lost touch with activities and people they used to enjoy, this is often where we can help most. We take the time to understand what mattered to them before, not just what is convenient to organise now

Usually Increased Social and Community Participation, or Assistance with Social and Community Participation under Core. We will check your plan wording with you.
No, and that is the point. Wherever possible we connect people to ordinary community life: a local club, a class, a place of worship, a sports team. Disability- specific programs are one option, not the default.
Then we try something else. Finding the right fit usually takes a few attempts and that is completely normal.
Where we can staff it, yes. We match on language and culture as a priority, not an afterthought.
